Prior to your new patient appointment, please have your prior clinic submit all records to us. We ask you to acquire formal medical records by signing a records release with them. You are also welcome to submit any of your own notes. Ask your prior center to submit a COMPLETE setting of records defined as including the following:
Clinic records
- All female pre-treatment diagnostics (AMH, tube test, saline sonogram)
- Any pre-treatment male partner semen analysis or advanced andrology testing (he may need to sign separate records release)
- IUI and IVF monitoring data (any lead-in medication, stimulation medication names and dosages, dates of office visits, all ultrasounds/follicle measurements and all estradiol and progesterone levels)
- Genetic carrier screening results (for both partners)
- All pregnancy testing and outcomes
Laboratory records
- Total egg numbers
- Semen analyses on day of egg retrieval
- Egg maturation rates
- Fertilization rates
- Culture outcomes
- Preimplantation genetic testing results
The more complete the information submitted, the better we can evaluate your case
Please verify receipt of all of your records 3 business days prior to your appointment. Additional records cannot be added to a second opinion after the review process has begun. Records can be transmitted to us by yourself or by your clinic via an email to [email protected] or faxed to 415.925.9410. Do NOT send to our patient portal (www.onpatient.com).
Please note that it is your clinic’s responsibility to send records. Customarily your clinic has 30 days to send them so please schedule your consultation date accordingly. Follow ups on the progress of your initial request for records should be made directly with your prior clinic.
